Step 1:

Find a comfortable sitting position

Take in a few deep breaths

Relax your mind and be as honest as possible when you answer following questions

 

Step 2: 

For the past 2 weeks,

how often did you experience the following?

  • Feeling nervous or anxious

  • Not being able to stop or control worrying

  • Trouble relaxing

  • Feeling easily annoyed or irritable

  • Feeling afraid that something awful might happen

 

Step 3: Results

If you have answered "almost every day" for

2 of the symptoms above:

"moderate anxiety".

All of the symptoms
